web based ILL

I am interested to learn what solutions libraries have successfully 
used to deliver ILL through user-driven web forms.

Are you using:
1. custom cgi based scripts?
2. iso 10161 things?
3. docline/oclc/???
4. 3rd party applications?
5. OPAC components?

I'd be particularly like to see examples of ILL service  webpages 
that you are particularly happy with.

I can summarize to the list if there's a lot of response.
