Circ & ILL forms using Google forms and script generated emails

Hi Shannon,

If you just need something super simple...I have used this script numerous
times to get Google Forms submission data in an email message:
http://www.labnol.org/internet/google-docs-email-form/20884/. Never used it
for circulation or ILL, but have for other kinds of requests.
