All -

We use Springshare's product that is part of their product line. I can't remember the previous name but it is now called LibWizard. Includes forms, surveys, polls, tutorials. We use the forms we created there to get ILL requests. Since it is a Sprinshare product, of course it integrates well into Libguides and other places as necessary. Output is to email if you want it to be. Plus the system keeps data.
   Here is our form: http://www.libsurveys.com/loader.php?id=79d6cce6af9bbd9eafca6b03c5a66c43

Hope that helps.

Subject: [WEB4LIB] Circ & ILL forms using Google forms and script generated emails

Has anyone set up circulation forms (such as document delivery) or ILL forms in Google forms and then created a script to email you when the form's spreadsheet is populated data, when a user submits a form response?

If so, would you be willing to share examples of how you set up the responses spreadsheet and the email script?
