In the name of full disclosure, I work for OurDigitalWorld (http://ourdigitalworld.org), a Canadian not-for-profit, supporting sustainable digital stewardship of cultural heritage.
We offer the VITA Digital Toolkit, which has a contribution module for gathering images and/or stories from the community using standards-based templates for description, geo-location, and more. It is a subscription-based tool, but very reasonable and offers a wide range of other options; also it's web-based, so there's no local hosting or dev work required.
