library 404 pages

We decided to experiment with the form to see if we'd get any feedback from
it and well... we started using it a couple months ago and no one has
submitted it yet. We were hoping it might at least prompt someone to say "I
was trying to find the [booklist that's no longer posted]" or anything that
might indicate a pattern, but so far no takers.
