Subject: Survey: QR codes in libraries

Hi all,

I am doing some research for a QR code presentation for my state library
association. We've put together a short survey to see how libraries (of all
kinds) are using QR codes and what kind of response they've garnered.

The survey should take 10-15 minutes to complete. We'll be collecting
responses through August 11, 2012.
