[Web4lib] library technologies -- baseline survey instrument

As a part of a grant the Five Colleges of Ohio is undertaking (cf.
http://www.ohio5.org/announcements.htm ) we've committed to creating a
survey that would...

"establish a baseline understanding of the level of library staff
knowledge in the areas of technology implementation and utilization
which are integral to next-generation libraries. These areas include
metadata standards; metadata creation and documentation; digital
scanning standards; creating, archiving, and distributing varieties of
digital media; social networking tools; web promotional tools; and
emerging classroom technologies."

If anybody knows of similar survey instruments already existing I'd be
grateful to hear directly from you and would summarize for the list.
