We've had a Talk Back feature available for the last couple of years,
and while it isn't one of the most popular pages on our site (.11 % of
total traffic to our site in the last year, 193 questions answered in
2.25 years), we feel it has been successful. We've gotten a lot of
useful--actionable!--feedback, and we have reconsidered and/or changed
some policies and services as a result.

We use a question/answer format, although a blog or forum might work
quite nicely. As another respondent noted, you don't always want to
get into a prolonged back-and-forth about an issue, and I think this
is reasonable so long as you take each comment seriously.
We use our SubjectsPlus [1] software to power this, so if you're also
looking for a subject guide management tool, the next version of
SubjectsPlus should have both Talk Back and FAQ "modules." Hopefully
available by the end of next week.
