Here's a shout-out for two books:

PHP and MySQL for dummies by Janet Valade

Yes, I know, I'm not supposed to suggest a Dummies book, but I think
they make a good introduction, especially if you follow them up with
their opposite. So, after Dummies, I say go with O'Reilly. The
O'Reilly books are so much better than their competitors, those
tiresome four-inch thick books.

I think Dummies/O'Reilly combination makes for a perfect math. You are
eased into some of the ideas in a non-threatening way, then you get
the straight deal. You skip the bloviation.
