[Web4lib] Free trials evaluation form?

Without trying to re-create the wheel, I'm hoping a few of you out there
already have a generic evaluation form for evaluating free trials of
electronic resources.

I want to make a form available for our staff to give feedback on new
resources.  We want to collect their input to find out if these things
really are easy to use, do they do what they promise, is it worth the money,
etc?  And other things people might not think of if we leave their responses
open and free form  ...

Anyone have a form they can share?  Thanks!
