This may be slightly off the topic, but maybe not.  When you get around
to doing your survey, I suggest you look at creating it using PHP
Surveyor.  We installed it here last fall, and it has become a great
tool for setting up surveys from the simple to the complex with very
little time or tech skills.  No coding knowledge is required to use it,
and best of all, it is easy to massage information out of the data
collected.
