Gathering data from an online survey

We are going through a website redesign and would like to put up an online 
survey asking users what they think of our website.  I know there is a way 
to get the responses we gather into an application such as a database or 
spreadsheet program.  I just don't know how to do it.

Would someone be able to briefly outline the steps involved?  Also, do I 
need any special software for this other than the application program (we 
are using MS Office).
