Summary: gathering data from an online survey

Thanks to everyone who responded to my question about dumping data gathered 
from an online survey into a database or spreadsheet application.  Here are 
the suggestions:

1. Use Microsoft Active Server Pages to connect an HTML form and a database 
(such as MS Access) that is stored on your server.
2. Use PHP and MySQL.
3. Use a commercial product such as:

         Surveymonkey.com
         WebSurveyor
         Zoomerang
