Hi Ed:
Without code or a url it is hard to answer your question specifically,
but I suspect that each of the forms or form portions or function names
have names, which upon duplicating are no longer unique. Review the
names, make sure that the they are unique and properly functionally
relevant to each other, and see if that helps.
