Internet Explorer Losing the Address Bar and Tool Bar

For the past several weeks, we have been encountering a problem with our 
public Internet PCs losing the Address Bar and Tool Bar. We have IE 
locked down using NT Policies and WinSelect. Right now we are guessing 
that some web site is trying to modify the default home page or install 
their equivalent of the nefarious Yahoo Toolbar.

Has anyone else encountered this? Has anyone figured out the cause or 
how to prevent it?
