Surveys on web pages?

We're thinking of using print and online surveys to gather information 
about our web site's usability. Has anyone out there in WEB4LIBland had any 
experience with this kind of survey? Do you have any words of experience 
for us?

If you'd like to share surveys that you've used at your library, feel free 
to respond to me (not the list!) with attached surveys.
