We are using a wonderful CGI script from www.arachnoid.com

http://www.arachnoid.com/lutusp/illform.html

they are careware, so it is somewhat like free :)

The forms are easily modified, and if you have a place to put the script you
are set.

Our usage of it can be seen at: http://depts.alverno.edu/library/libreq.html

I am interested to learn what solutions libraries have successfully

Are you using:
1. custom cgi based scripts?
2. iso 10161 things?
3. docline/oclc/???
4. 3rd party applications?
5. OPAC components?

I'd be particularly like to see examples of ILL service  webpages 
that you are particularly happy with.
