Request for information on copyright forms in use

Colleagues,

The California State University Monterey Bay Library faculty are
interested to learn of any copyright release forms which your Library or
institution uses for student or faculty created work.  We’re interested
specifically in release forms pertaining to work which your Library makes
available via your electronic collections. The CSUMB Library is developing
a set of guidelines for senior undergraduates to use when submitting their
senior projects (many of which are in digital or other non-paper formats)
to the Library for archival online access. These guidelines will include a
statement of copyright release, which will clearly state the author’s
willingness to allow the Library to provide online access to the works to
a defined set of users (e.g. the campus community).  We would like to
consider the variety of copyright release forms which may already exist.

Additionally, we would be interested to read any similar author submission
guidelines your library may use for accepting student work  theses,
dissertations, senior projects, etc.
