ILL web request forms -Reply

As a P.S. to the message below that I posted Monday, I want to let
everyone know that there is a web site that contains links to many
ILL web forms.  It is located at:

     http://hertz.njit.edu/~robertso/LibForms.html

Many thanks to the folks who pointed this out.  I am finding,
however, that many libraries are sending me sites that are *not*
included there.  I will be happy to continue to accept your listings
if they are not already on that page.  I will post the summary in
about a week.

I will be giving a presentation on this topic at an Oregon Library
Assn. roundtable conference soon, and wanted to share a list of sites
with the attendees so they can see all the creative work our
colleagues have done. No use re-inventing the wheel!

     Does your library use a web page form to allow your patrons to
submit interlibrary loan requests to you?  I am compiling a list of
these ILL web request forms.  If you know of a good, functioning
form, please send me the following information:

      1.  url address 
      2.  library name 
      3.  3-letter OCLC code if applicable.

     If you send your submission directly to me, I will be happy to
post the compilation to the list.  Many thanks!
