Request for Web censorship forms

Hi folks - Does anyone have any electronic forms that they have people 
fill out if they want to complain about materials they find on your Web 
sites?  This would be like a request for censorship on the Web - or 
something like that?

We recently had a case here where a student put some photos on his Web 
page which an administrator wanted censored.  In the future we'd like to 
have a policy in place for when this happens again.  We think that the 
first step would be to have a form that complainers can fill out similar 
to the ones we've had for libraries.  

Any good examples out there?  Please let me know!  Thanks.  Jerilyn Veldof
