Censorship requests for Web site

Hi folks. Recently an administrator at the University brought to our
attention that a student who had recently won a "Best HomePage Award"
on the University-wide homepage had some really racey, "indecent" photos on
his site which she thought should be taken off. As a result the UAInfo
Council decided we better get a procedure in place to deal with this kind
of thing. I suggested we create a form for complainers to fill out and state
their issues which the council would then route to the appropriate office (i.e.
Affirmative Action). We might model it after the forms libraries have
been using forever to deal with censorship requests.
Does anyone have this kind of form on their Web sites which I could look
at? In what other ways have folks dealt with censorship requests on the
Web? Do you delink homepages or do you go as far as kicking people off
your servers? Does anyone police their pages or do you
just wait to see if someone complains? Then what procedures do you follow?
