Creating interactive web forms

Hello!

I'm new to this listserv, and I hope that reaching out in this way will help me stay abreast of the best emerging technologies for libraries.  Also, I need to pose a question to this community!  My library is in the process of creating interactive web forms for typical library requests like interlibrary loan, instruction request, instruction evaluation, research consultation, etc.... I am looking for a good web form builder, but my search is complicated by the fact that we have lots of specific functions we want our forms to have.  For example:

1.  Multicolumn questions (instead of just one question per line in one, long column, which is like many of the open source form builders).

2.  Form must have query-capable database "behind the scenes" where all information is collected and displayed.

3.  All submitted information should be able to be in a printable form, or be converted to one.

4.  Forms should be web-based and preferably should email creator of form when form is submitted.

I have found many open-source and pay form builders but have yet to find one that has ALL of these functions.

Can anyone please pass along experiences and/ or recommendations for form builder sites to me?
