Or, try Google Doc Form if you have gmail account for many reasons. 
 
It is easy to create a form (literally no HTML code knowledge needed)
It automatically inserts to a spreadsheet.
It allows you to download the spreadsheet on your local machine. 
It can also set to notify you when a new submission comes through.
It allows you to share the document with your colleagues online.
It has summary feature (not perfect but nice)
 
The only minus side that I can think of is - you might still work on it at home.

Or that gets written to a CSV file on your server, which you can then
download and import into Excel or Access....

Of course, the problem with forms is preventing spam from getting in,
but it can be done.
