Squidguard fits the description for all of these.  We
are using it with success, and are testing it as an
enterprise filter for the 18+ libraries we provide
Internet access to.  You can find out more about
squidguard at http://www.squidguard.org/
It is technically a redirector, but the difference is
mute.  It runs very efficiently, and you can fully
modify the blacklists which filter several different
catagories.  It is wonderful!  And of course, Open
Source.
