[WEB4LIB] Web-based polling/survey software

I run an alumni home page for my high school and I've recently found
www.formsite.com which allows you to do forms but I also believe it has
templates for polls or surveys.  You can use it for free if you don't mind
the ad (I haven't found it particularly intrusive) but you're limited on the
number of entries, or you can pay and get unlimited usage without ads.
