Employment Forms

One of our departments would like to take employment applications over the
Internet.  I haven't personally used one of these forms but I know that a number
of sites that use them.  I'm exploring a couple of possibilities including an
HTML-form+CGI script and a forms-enabled PDF file with Javascript.  I would
appreciate any "good" examples of job application forms that you either support
or have utilized [and "bad" examples too if you've seen something that should be
avoided!]  If your library allows job applicants to submit their applications
online, I'll be particularly interested in your experiences, especially with
integrating the submitted information into your information processing
