At Caltech, we've recently migrated from a home grown set of web-based
document delivery forms to a commercial system.  The home grown system
used cookies to record requester names, contact information, etc to cut
down on the repetitive typing.  The cookies were deliberately not set
within the IP ranges of public workstations on the library subnet or in
the campus computing labs.
