Library web site policies/guidelines ClipNote

A call to all academic librarians on this list that are at 
Institutions with an FTE between 1,000 - 7,500.

After looking for library web site policies to help me write ones for
my library, I developed an interest in this topic that has led me to
the idea for a CLIPNote.  Now I need to do an infomal survey to 
submit with my preliminary proposal--this is one of the methods they 
suggest for gathering data.

I am interested in library specific guidelines/policies only. I 
would like to know if your library has standards and guidelines for 
those persons creating web pages for your site. If so, could you 
share this information with me?

For those of you who DO have policies...when did you create them? 
At the beginning or later on?
Who were they written by ?
How many people create pages (design & code) for your library?
Do you have a library web site committee? Who are the members? 
Number?
 How often are they revised? 
What are the components of your policy?
Do you also have guidelines for certain types of pages? 
Bibliographies? Specific library departments?
Do you maintain your own server? If not, do you have direct server 
access?
Does your college or university also have policies/guidelines 
you must also follow?
Do you think policies/guidelines are important for a library web site 
to create & follow?
 Do you have policies regarding Internet use in 
your library, or a collection development policy regarding Internet
resources?

If you do NOT have any library specific policies or guidelines...why
not?
Do you feel that your College or University has policies &
guidelines that you can follow & are adequate for your needs?
Have you not had the time or staff to create them?
Do you wish you had them?
How many people create pages (design & code) for your library?
