Web-based surveys or exams

Hi all-

I am looking for models of online exams or suveys and for the perl 
scripts needed to process them.  I am still a Perl novice and I am hoping 
that those of you further along the Perl path can either let me know of 
scripts you yourself have created that you might be willing to share, or 
point me to a good archive.  Please respond to me directly --
