patron Internet surveys

     I am not part of a library, but I recently encountered a set of 
     internet usage surveys I noticed included a number of questions that I 
     would like to ask.  The site and the questionnaires are located at 
     Georgia Tech:  http://www-survey.cc.gatech.edu  (the survey runs until 
     Nov. 10)

Our public library is planning to survey patrons regarding Internet use, 
feelings about full access, types of Internet activities, time limits,etc. as 
part of the efforts made  to revise our Internet policy.  I would be 
interested in knowing if other libraries have done such surveys and if such 
libraries would be willing to share some of the questions used in the surveys.
