Assuming NCSA's HTTPd server, see:

  http://hoohoo.ncsa.uiuc.edu/docs/tutorials/user.html

Assuming CERN's/W3C's HTTPd server, see:

  http://www.w3.org/pub/WWW/Daemon/User/Config/AccessAuth.html

For other servers, consult your documentation.  There isn't a generic way 
of doing this, especially on platforms other than UNIX.
