Abuse of Forms?

I'm curious to know if those of you using forms on your library's web 
pages have experienced any "abuse" of the forms--obscene or abusive 
messages to library staff, frivolous book purchase suggestions from  
non-campus folks, etc. 

Any precautions you'd recommend? 

Also--How does your library verify that messages sent from your web pages 
are actually submitted by the faculty/student they claim to be from rather  
than by someone who's simply changed a browser's mail preferences or put 
someone else's name on a form?
