[WEB4LIB] Do you keep library application forms?

Great to see a fellow Okie on the list!

At our library, Tahlequah, we have a seperate form for Internet access.  In addition to filling out the library card enrollement form, if the patron wants access to the computers, they must read and sign the district's (EODLS) user agreement and policy sheet; then we issues those patrons a special card or put a sticker on their existing library card.  We keep the library card enrollment forms for quite some time before periodically shredding handfulls at a time, starting with the bottom.   As for the Internet user form, we keep those in binders, in alphabetic order, forever; we don't destroy them.

Gotta run and teach a class!

  One of our Library Board members (he's a lawyer) has suggested that we keep library card applications (we currently toss them when a card is successfully issued) so that we have a record that the an adult has signed the application and acknowledges that a library card brings with it access to the internet.
   
  Does any other library do this?  Do you see a need to do it?  Thanks for any info you can provide.
