Need form for authors entering cites for bibliography? Help.

I am wanting to add a web-page form for entering citations by authors.

It could be emailed to me, but possibly input into a section of the web page 
bibliography (in any location for later enhancements by me)

I need a form and cgi file to give me a jump start.

Does anyone have one or know where I might look?
