Thanks to everyone for their quick replies!  Yes, the problem was on the
server, I needed to map the MIME type.  For Microsoft's Internet
Information Server, you need to launch the registry editor (regedt32.exe)
and edit the
HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Services\InetInfo\Parameters\Mim
eMap key.  You need to add "application/w3launch,w3l,,5", and then stop and
restart IIS.

This immediately fixed the problem with the demo files I was using.  I have
tried creating my own .w3l file for Phone Disc, and I am getting "Unable to
find group title in local files".  I suspect that this means that the group
title in the pdwin.w3l file is not the same as the group title in the
w3launch.ini file - but in fact they are the same.  Could this mean that it
is having a problem finding the pdwin.exe file?  Once again, I am stumped.
