Need Web based Reference and Online Search form URL's

1.  I need some examples or suggestions for WEB based Reference request or
Online Search request forms.  Can you all help by pointing me to some URL's?


2.  National Library week is fast approaching.  Are you institutions doing
anything special to promote WEB based Library/Information activities during
this time and would you all mind sharing?
